''Temporary Custody'' legal paper drawn up if leave child with a friend for a we

Our family is planning a vacation and wanted to leave our 5 month old baby with a friend for a week, someone told me I would need to have a ''legal'' paper drawn up and signed in case the child would need medical care, etc that this person can give her consent in our absence. Is this true? Where do I get this legal paper, etc, what is the law? We want all basis covered in case of an unforseen emergency. Thank you very much.
