Suing a governmental agency

There is a government agency that I believe made me loose money. What forms do I use to sue this agency?

There is no "form." Hire a lawyer who will file the suit for you, if you have a valid case. You didn't say how the agency did this. Many acts of governmental agencies are immune from suit. You need to talk to a lawyer.
