Qualified Domestic Relations Order

In my devorce degcree a retirement account through my ex-husband's work was overlooked. My ex-husband and I have discussed the account since that time. Since I am disabled and unable to work...we decided to transfer the account into my name only. To do so we were told to remove my ex-husband's name we needed to submit a QDRO form. Does an attorney need to do the oder or can we submit ourselves? If so, where do we get the form to do so?

You may not need a QDRO. First, contact the company that administers his retirement account, they can tell you the procedure to do this. IF they need a QDRO, then they may help you or provide a form for you. IF they don't, I would suggest getting an attorney. QDRO's are great tools, but they can be very technical and difficult.
