In the state of Arizona, if requested, must an employee provide a doctor's not for missing one day of work? And, if so, is the employer responsible for the medical bill if they are requiring the note?

Generally, an employer can require an employee to bring in a doctor's note. The employer is not responsible for paying a medical bill simply because the employer requested a note. Thank you for the inquiry.
