ADA access for individual traveling with a companion dog

I was told by my Dr. to get a dog as a comfort and support animal for my agoraphobia. I was interest in what proof I need to support his request when out in public, e.g. dining or traveling? What is required to satisfy anyone that requires proof of this condition. He has given me a letter regarding my need to be accompanied by a pet.

IF you qualify for a 'service dog' under the law, then you'll need to get such a certification from your doctor that is in compliance with the state and federal laws, then get an animal that is trained and certified as a service dog by your appropriate state and local agencies [it varies by location], get the training and certification for you that is required for you to use the service dog, carry with you at all times those certification documents, have the appropriate distinctive vest and harness with appropriate markings and warnings for the dog. Then you are entitled to take the dog almost anywhere in the US; you can forget overseas travel with it or any other animal.

Now, if instead you mean you want to have a puppy that gives you comfort, you'll just have to plead for sympathy for your situation wherever you go, and no, most businesses, restaurants, medical offices, airlines, etc., will not likely allow an uncertified dog entrance.
