Reasonable Accommodation denied

From my interpretation of discrimination policies a request for reasonable accommodation does not need to be formally reqested. How specific does one have to be, when requesting a reasonable accommodation?

Sufficient to convey the need and medical basis and doctor's opinions. They aren't required to read your mind.
