I have a house painter that will agree to sign a waiver of liability in case he gets hurt on the job. Where can I get such a form for CA.

Try the Sonoma County Law Library. The staff can assist you in finding a forms book that will have possibly-useful sample forms, and they have copiers and computers you can use. It is in the courthouse area, on Ventura Drive, I believe.
