Need a legal agreement form

We want to lend our new tool to some companies in exchange for using their results/testimony and company info on our website for promotional purposes. what is the name of the form I need for this arrangement? We would also do a NDA form.

"Name" of the form? You need a written agreement specifically tailored to the needs of both sides, protecting each from the risks they may or may not understand. A "do it yourself brain surgery" approach to this is certain to result in legal problems in the future when a dispute arises that should have been covered by the agreement. No such form exists. Forms are created for repetitive use, such as filing taxes, registering at hotels, issuing parking citations, and even some legal filings, for which the Judicial Council has promulgated optional or mandatory forms. However, there are no forms for non-routine business deals. Unless substantially the same thing is going to happen over and over, at least say 100 times, like an offer or residential real property or application for a driver's license, who would bother to write up, publish and distribute the form? How would they be indexed and found?

What you need is a situation-specific contract. Some experienced business people could write their own with no serious side effects. Otherwise, the agreement should be drawn up by a lawyer who has listened to one or both parties carefully describe the deal and the intended results.
