While you might find something that looks appropriate on the internet, you'd be much better off having a well-crafted, customized collaboration agreement drawn up. It's not a tremendously complex or time-consuming document, but there are nuances that a lay-person could overlook. Having a lawyer draft it is the smart move.

A collaboration agreement is like a prenuptial agreement between co-creators. The work you do as collaborators is like a child. It's worth it to plan the "what-if" scenario early on, so custody and support issues are resolved before there's a problem.
