is it legal for a doctor to ask for your religion on a medical intake form?

I don't see anything improper about doing so, or your declining to answer the question. I suppose that, if your needs include spiritual, the doctor conceivably could summon a minister from your chosen religious affiliation.

Medical facilities regularly do so they can provide a priest or pastor if needed.
