Legal Question in Credit and Debt Law in California

Debt Collection

I received a summons because i am being sued by one of my credit cards.It says that i havent been paying them,but i have.I am enrolled in a debt consolidation program and they send me a copy of all the payments they have made to the credit card.It also says to send a typewritten response in proper legal form back to the court were this case is filed.What is the proper legal form to use?I dont have enough money for a lawyer. Thank you for your help.

It is called a general denial and you can get one from the court clerk.
