filing forms/motion in court for expungment

Yes, I need to know which forms I need to file a motion for expungment for a misdemeanor violation that took place 10 years ago and is still appearing on my record. I was told I could easily file for expungment, but I don't know what forms I need or where to get them exactly. If there is any other information on this suject that you know of with this type of problem/ matter/situation that may come up that I need to know about, I would deeply appreciate that information as well.

A simple PC 1203.4 form which is avaialble at the Court or local Public Defender. It is filed in the issuing Court and usually requires a fee to be paid.
