What form/letter would a doctor need to fill out requesting an inmate be released to take care of his mother

Nice try. In order to accomplish what you seek, a declaration from the doctor would have to be made part of a real convincing motion to the court for such temporary release under a modification of its orders. It's unlikely to succeed, but if you're serious about trying, feel free to contact me if the case is in SoCal courts.
