Forms? You'll have to try to issue subpoenas for them, serving all the required notices on the phone company and the customer, and then deal with their legal Opposition to the subpoena. If you don't know how to do so, you'd be advised to hire an attorney to help you.

You would have to have the court issue a subpoena, and give appropriate notice to the consumer. This is a complicated process. Is there a reason that you do not have an attorney in a criminal case? I mean, you have the right to represent yourself, but if you cannot afford one, they give you one for county expense. I know Ted Bundy represented himself, and got the death penalty.
