violated probation 4 times and now have a F.T.A and would like to have proper form to file with court to plea my reasons and avoid jail

There is no form for this purpose. You could submit a declaration to the court stating the reasons for your probation violations and FTA. This declaration would be served on the probation deparment and district attorney, at the time it is filed with the court. This would give the probation department and district attorney time to verify your statement. Most defendants appear in court with their attorney and then have the attorney plead their case. Unless you have a really compelling reason for the failure to comply with probation and appear in court, and a sympathetic judge you are probably looking at some jail time. Being represented by an attorney will give you a better chance of avoiding or at least reducing jail time.
