Can I apply for conservatorship of my grandmother if my cousin has power of attorney? She is not caring for my grandmother properly.

Yes. However, there are a number of significant legal issues that you will need to discuss with an attorney. First, if your mother has designated this cousin as her first choice to be conservator, the court will honor that choice unless there are significant grounds not to. Secondly, the Court will appoint an attorney to represent your grandmother to find out what she wants. Finally, the Court will also evaluate who is the better person for the job and whether you are capable of assuming the huge responsibilities that come with the job.
