How do I optain a conservertorship for my mother who has end stage dementia?

By filing all the appropriate Petitions and supporting pleadings in probate court, and having a hearing on the matter. You will likely need an experienced attorney to help you, as the proceedures are 'nit picking' and detailed. Contact me if interested in doing it right.
