Is there a legal avenue for forcing an 87 year old alcoholic into a

treatment program even against their will?

There is, by using a conservatorship. The court can appoint a conservator to make medical and financial decisions on someone's behalf if it finds the person is unable to do so on their own. However, it can be a difficult and expensive process, so you may want to start with a call to Adult Protective Services first to see if a solution can be achieved without the courts.
