my sons father has agreed to give up his parental rights, what forms do we fill out for the courts? He lives in LA county and I live in San Diego County.

You should consult with an attorney about this issue. Unless the child's father is asserting some right to visitation nothing is at issue. Your child may want to have his/her right to inherit from the father and to seek support in the meantime. You may not be acting in the child's best interest in the long term. However, the facts you presented do not indicate whether there are any benefits from formalizing your child's surrender of his/her right to maintain a relationship with the father. Don't act prematurely without a thorough evaluation of the benefits and drawbacks of your actions.
