Legal Question in Immigration Law in California

Hello, my grandmother recently lost her Certificate of Naturalization and as such I am helping her file a N-565 to replace the document. The only problem is she does not know her Certificate Number or A# nor the office in which she received her first Certificate, all of which, is information requested by the N-565 form.

My question is if this information is necessary for filing this form as she does not know it or will if its possible to leave the spaces blank and instead provide a copy of her Social Security Card and Driver License for Proof of Identity.

Yes, she needs to provide her A#, the number of her certificate, date and place where it was issued to her, and supply a photocopy of the certificate itself (if available).
