I quit previous employment due to stress related reasons and i need documentations that could be signed by witnesses and doctors what documents and how?

Talk to a Workers Comp attorney if you think you're going to pursue a stress claim. He can advise whether you have a case worth pursuing, and what it will take.

If you thinking of a 'wrongful termination' or 'discrimination' type case, you'll have to prove actual illegal conduct by the employer, not just stress and bad working conditions. They're not illegal.
