Legal Question in Legal Ethics in California

My hospital in California is requiring we sign a form if we decline the H1Ni vaccine. The wording of this form are statements I do not agree with. Specifically some of the bullet concepts that state " I cannot get the Influenza disease from the Influenza vaccine" Another is "The consequences of my refusing to be vaccinated could endanger my health and the health of those with whom I have contact, including: patients in this healthcare setting, my coworkers, my family, my community.Despite these facts, I am choosing to decline." We are being told we MUST sign this and are being pressured to do so. Is this legal, or can I refuse to sign. Thank you

You can "X" out and initial the parts you disagree with, and just sign the area that says "I decline the H1N1 vaccine" or write it in.
