Prior to receiving an Involuntary Lien, should I have received some type of "proof of service"?

If there was an underlying court judgment, and you never got served with a summons and complaint, find out the case number and ask again giving all the facts.

It depends on what the involuntary lien is for. I can't tell if you are complaining about a tax lien, a mechanic's lien (which is really consensual) or a judgment lien.
