do you have to stay 30 days with employers physision or doctor if he does not modify your work after a back sprain or other complacation that could become permanant if not treated

Yes, you are not allowed to select your own physician for thirty days, unless you specifically contracted to do so when you were hired by your employer. After thirty days, you may select your own physician. The rules change if you have an attorney representing you. You can call the assistance officer for free and receive timely and correct information.

Generally the employer has control of the first 30 days of the injured worker's medical treatment. However, if that treatment is inadequate, the worker need not suffer. You would do well by obtaining representation so to be referred to an applicant sympathetic doctor. The worker's compensation system is very adversial. The insurance companies will get away with anything they can. They will pay you the minimum on your claim unless you obtain representation to protect your interests.
