How does one garnish wages from a self-employed contractor in Colorado? What form is used?

Problem: I bought a foreclosed home and the lender required a licensed contractor to complete the necessary repairs within 90 days before I could move in. The lender issued a check to the contractor to begin the work. The contractor received the money and never did any work. I took the case to court and I now have a judgement against the contractor. The clerk told me I can serve the contractor with the interrogatory form and the wage garnishment form at the same time. When I asked the clerk what garnishment form I should use (business or individual), she said ask a Lawyer!

Please help!
