Fact Information Sheet

Plaintiff is refiling a 20 year old judgement that is about to expire along with new legal fees. I have been given a Fact Information Sheet to fill out. Am I required to fill out the spouse related portion if she was never a party to the original judgement?

I am also required to submit a Notice of Compliance with the court. Is there a special form that is used or do I submit a copy of the info sheet?

I would not fill anything out unless you seek counsel. Also, I'm confused about re-filing a 20 year old judgment. In Florida, judgments are good for 20 years only.
