Moving a minor out of the country

Divorced in 2000, primary custody of minor son was awarded to the the mother. She moved to Canada in Sept of 2004 and left the child with me supposedly as a temp situation to care for her sick father but has not returned and is no longer living with her sick father but has moved in with a man she met there, nor has she at any time offered to contribute child support. Recently she has stated that she intends to move our now 13 yr old son to Canada . As a normal confused teen , he has also stated a desire to move there, my concern is, if I allow her to take him out of the country am I relinquishing all my parental rights , what should I do legally to protect his rights as well as mine ?

Seek modification of the prior order if it is in the best interests of the child.
