You sign it in presence of a Notary Public, and have a licensed lawyer's assistance in preparing, if help is needed.

A Notaty Public cannot prepare an Affidavit for you (unless he or she is also a licensed Attorney). A Notary can only notarize an affidavit (put his notary seal and sign after you sign it).

C'mn you guys -- how old are you? The best place to get any affidavit sworn is before a notary, which you can obtain at any bank in the United States. They are required to have a notary on staff. You can always get a notary at a U.S. Embassy if that is necessary. Good luck!
