Yes, you can change the entity from a corporation to an LLC, but there are legal and tax consequences to your change. Therefore, you should consult your tax advisor and an attorney to obtain full information about the matter.

I'm not sure if you mean changing the name or the entity type (LLC vs. corp.), but either way you can do both. You can convert a corporation to an LLC, and vice versa. A company name is changed by amending the articles. And technically, changing the entity type will change the designation and thus the name as well.
