can a hospital garnish wages for a bill that had previously established payment arrangements and those arrangements are being upheld?

Unless you signed a consent judgment or other document that permits garnishment, the hospital has to obtain a judgment against you before it can garnish your wages.
Also, the payment agreement should have been in writing. Otherwise, creditors tend to play games with people by purporting to enter into an agreement simply to make some money. Thereafter, they sue the person anyway (even if the person is unwittingly complying with the verbal agreement).
If your outstanding balance is significant (over $5,000), you should consider hiring an attorney to advise you on the matter.

If the debt is large see a lawyer not only as to what you asked but also to see if wiping out the debt via bankruptcy is an option.
