If we had a paternity test done, which proved that I was the father, do I have the right to visitation and to give that baby my name?

No, a paternity test is evidence that you are the father. You need to legitimate the child so that you can get access rights for visitation and to give the child your name.

You need to file for legitimation to gain rights of visitation or custody or to seek a name change.
