I placed my son father on child support, and he was ordered to provide medical insurance or pay 69% of the medical bills and he did not do so what so ever. Can I take him to small claims court. Because child support is telling me since he is self employed the medical cannot be enforced

You do not take him to small claims court. You take him back to family court and have him held in contempt. His wages cannot be garnished as he is self-employed. However, the court can still put him in jail if he does not pay. Your arrears have to be several thousand dollars though. Child support enforcement will assist you once you meet this threshold.
You should have filed a motion for contempt in court. This way you bypass Georgia child support agency. If he continues to defy the court's order, you simply return to court. The judge will put him jail until he complies.
