I recently filed a temporary protective order and received a court date and am wondering what, if anything, I should take to my ex parte hearing. I was told that I should have the motion for ex parte relief with me. Does this mean I should bring the blank form that the judge fills himself that I can find online and bring it with me? Or would it be better to draft up one myself, using a template from a previous ex parte filed by a lawyer against defendant on my behalf?

Given that your safety and finances are at stake, the best answer is to sit down with a lawyer. A TPO may not even be the sole thing you need to do, so doing this without legal advice is dangerous.
Glen is absolutely correct, especially if there are children involved.
