getting out of an apartment lease
I want my name removed from lease with roommate so I will not be held liable for her misconducton the property or future financial responsibilities incurred by her or her future rommates.

A lease is a contract. You can't just walk away from a contract. So unless the lease has an early termination clause, you are stuck.
If the landlord agrees to release you, and you can ask (and the landlord has the right to refuse), if you get a yes, get a very specific release in writing and keep that release at least 20-25 years.

Ask the landlord to release you, if you have moved. Since he/she likely has no incentive to do so, it may not happen, but that is usually the only way.
