My husband fell off his bike riding home because an apparent rut in the road. A car then stopped on his arm? What can we do? We filed a claim with her insurance but they want to deny the claim?

Your post is far from clear, starting with who you expect to pay and why? If your husband was riding his bike and there was an obstacle in the road ("rut," pothole, grate, or whatever), why was it not his fault for looking where he was riding and avoiding the accident? If not his fault, and an unavoidable accident, why is anyone else liable? Roads are full of obstacles to avoid. As far as the car, you provide no details on why that driver is liable or how that happened. Finally, you don't identify what insurance, and why you believed they should pay (if you so believe). In short, most of the story is missing, starting with the basic question of why anyone other than your husband or his insurer should pay or do anything.
