notice of lien
We want to place a lien on property where building materials were supplied by our company but the payment was never received. Would you, please, tell me what is the form that I have to submit to our customer. And would it be possible to get a sample of that form.

I would suggest that you approach a business attorney to accomplish this for you. Many times there is a lot more involved than filing a form, which is, in many cases, simply a notice to lien.

You may want to talk with the clerk of the county recorder's office to obtain the form, and ask how much money for recording the lien. This is not legal advice as you are not as yet my client.
