Legal Question in Family Law in Illinois

When I was born my birth certificate/last name was the same as my biological fathers laat name. Than at age 3 my mom remarried and the new husband legally "adopted" me which changed my last name as well as birth certificate to the same as his last name, the step dad. Teeny six years later I had my name legally changed back to what it was in the first place, my real dad's last name. My ID, my SS card, bank cards, everything has my birth name on it now except for my birth certificate, because I guess when my mom remarried and had the adoption put through they cjanged my ORIGINAL birth certificate. How do I change it back?

My step dad was NOT a good person and I want nothing to do with him or his last name please help ?

I do not think your name change provides a legal basis to change the name on your birth certificate. If your real dad adopted you, that might provide a basis.

If you legally changed your name back to your original last name, you should provide a copy of the order to the bureau of vital statistics in springfield, illinois. the order should provide for the issuance of a new birth certificate in your new name. you can call me at 312-372-5600.
