Legal Question in Family Law in Illinois

A decision was granted for temporary custody of a 14yr for the mother to move out of state. Now the child has returned to IL to live with the father and wants attend school here in IL. The mother has consented to this request.

The new school district that the child will attend, will not allow enrollment unless the father has documentation of this development.

Question: What documents can be obtained to switch custody?
