Legal Question in Family Law in Illinois

Domestic relations court in cook county. I was given 28 days to respond to a motion from my ex wife's attorney. I need to know what is the correct paperwork/form to file, I filed an appearance but her attorney told me that was not the correct form that I need to file a response but not sure how to do that.
