Legal Question in Family Law in Illinois

My niece is voluntarily giving me custody of her 6 year old son. Is a signed statement from her acceptable for schools and medical care? If not, what kind of legal document do I need?

The proper term is guardianship, and there is a form you can have filled out that gives you temporary guardianship if everyone is in agreement.
