What does "If no action is taken on or before the below date the cause will be dismissed for want of prosecution, on motion of the court" mean?

The judge may dismiss the case on or after the date if no one does anything by that date.

It means that because nothing has been going on in the case according to the court's docket for some period of time, the court will dismiss the case becuase no one is prosescuting or proceeding.
