Preparing for Death

My Client is dying of cancer, she is married and owns her home in Severalty. At the time of purchase her foreign husband did not have a social sec #. She wants to make sure the house (and mortgage) will go to him and not into probate. What does she need to do? Quitclaim Deed to her husband? What about assuming the mortgage note?
