my medical information
Is it illegal to take your medical file from your doctor's office if you no longer wish him to be your doctor? Since it contains your personal medical information, whose property is it considered?

The information is yours; the physical records belong to your physician.
Under KY law, you are entitled to one free copy of your medical records.
Also, if "Dr. A" is your current doctor and "Dr. B" is going to be your new one, Dr. B may be able to request and receive your original records from Dr. A as a matter of professional courtesy.
