Legal Question in Civil Rights Law in Louisiana

Hello,I live in the state of Louisiana. I have a civil matter to ask about. I need to know on how to bring a civil case back to court and to represent my self. Its on a court order visitation and support for my underage son. The other party is not letting us see my sons daughter again and I need to know on how to do this on our own since they do not provide forms for contempt of court. Please help me.

Find a RULE TO SHOW CAUSE form and alter it to be a RULE TO SHOW CAUSE WHY "NAME" SHOULD NOT BE HELD IN CONTEMPT OF COURT
