can an exspouse still write me in as guarantor on childs medical bills, even though I do not carry the medical insurance and they have custody?

As the parent of the child, the medical provider can come after you for payment of outstanding bills. Any agreement or court judgment between you and the other parent has nothing to do with the medical provider.
