Using Medical Testimonials
I am a chiropractor in Maryland and would like to use patient testimonials in my advertising. Is this legal and if so, how should I phrase the document that I ask patients to sign giving me permission to use their statements?

A properly presented testimonial can be used in advertising.
The copy of the advertisement must be an accurate representation, presentation, descripton and depiction.
You may want the form and content of the testimonial advertisement reviewed by an attorney.
A release for the testimonial itself is needed.
