Which parent is responsible for a medical bill
... the parent who has legal and physical custody?
or the parent who took the child to the doctor/urgent care without first conferring with the custodial parent?
My husband's ex-wife took their daughter to urgent care during her visitation time. She did so with my husband's knowledge. She signed the papers at the urgent care that my husband was the responsible party - without his permission.
The urgent care company sent the bill to my husband and refused to accept that he was not legally responsible.
Who was responsible for the medical bill?

The first place you should look is your divorce and custody decrees. Usually, responsibility for medical bills are laid out in those documents.
