Legal Question in Criminal Law in Maryland

I have been sentenced to do 18 months in DOC in Jessup, Maryland. I am to report to the Harford County Detention Center on September 28, 2009 for transport to prision. I need to find out the proper format that I would write to the judge to request a modification hearing either prior to entering prision to see if the judge would take the time back or after I already am there. Could you please send me the proper format on how to do that. The jurisdiction is Harford County Maryland.

People use different formats. A motion for modification and reduction of sentence must be filed in writing with the clerk of the court within 90 days from the day the court imposed sentence.
