I was arrested for DUI and was never read and never signed the DR-15 advice of rights form. The officer never checked the box saying he read me the form. I was unaware of the consequences of blowing into the machine. Will my case be dismissed?

The rights form only plays a significant role in your trial if you did not take the test. However, since you can lose your license to drive in a DUI case, you should not go to court without a lawyer (even a public defender if you cannot afford one).
