my son and his xgirlfriend have a 19mon old baby together she has physical custody by the court but the baby has been living with my son and myself 3mon and hardly ever seeing her mother. what would be the best way to go about getting full custody of this baby

The best way to get full custody of a child in which there is already a court order giving custody to the other parent is to file a petition for modification of custody.
I agree with the first answer. You should hire an attorney and file for full custody with limited or no visitation. Of course, your son should also request child support, just to require that she take some responsibility for the child.
